Send it to the same address as you are having your assessment at.
You should also take a copy on the day and ask the assessor to read it before they proceed with the interview.
Finally if you can afford it, send a copy to the DWP office dealing with your claim, marked for the attention of the Decision Maker.

I'm sorry I didn't reply sooner, I sent it to Wembley (Maximus), Do you have the address? Let me know if you need it.
Please make sure you send it recorded delivery. When I phoned 10 days after sending mine in they said they hadn't received it, until I said I had a printout of the receipt for Royal Mail's delivery with the signature and name of who signed for it. Then they found it, so it's really important to get a receipt for EVERYTHING you send them.
I have also been told to take a copy with me.

Is : 501 sure that Maximus Wembley would be the correct office to send their ESA50 to ?
If they are unsure, they should check with Maximus :
Contact The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15.

Thank you for the replies, the original letter is from Wembley, but my assessment is in Luton.
Should I send it recorded delivery to Luton.
