Hi 501,
Personally, I would check for the correct postal address with Maximus, (CHDA) then post it recorded signed for.
I would also take a copy along to your face to face at Luton and hand it to The Assessor before the start of the assessment.
Have a look at this FAQ : ESA Medical – What to expect
Remember, how you manage the return trip to and from The Assessment Centre will be taken into account as part of your assessment.
Observations of you may also be made on approaching and leaving the building and whilst inside.
Assumptions may be made as a result of said observations.

Just been in touch with Maximus to double check where to send my ESA50, because my Wembley letter had a po box number on it, and have been given a Wolverhampton address, is this correct.

Hi 501,
Wolverhampton is the main Mail Handling site for all national DWP mail, so this could include Maximus.
It is sorted at Wolverhampton then electronically sent to the appropriate office.
I would post it to the address that you have been given, and take a copy to the "medical" as well.

Hi.
My assessment is not in Wembley but that's where I sent the ESA50, its just a mail handling centre for the forms.
If they haven't sent you a new ESA50 you could do what I've done, send one to Wembley and then take a copy to the assessment with you.

Update....
The medical evidence I submitted was not good enough and they still insisted I attend the F2F.
Then they phoned and cancelled my appointment two hours before I was due to attend my F2F, they said the nurse was ill and they had no-one to cover her. My partner had taken the day off work to take me as I cannot go alone. Now he has to book another day off for the new appointment in June.
As I am so untrusting it went through my mind was it a test as part of the assessment to see how I reacted, as I stated on my form I don't cope well with last minute changes
If they have said my medical evidence and the new ESA50 is not good enough, I wonder if they have already made up their minds before I even get to the F2F!

The staff who decide whether an face to face is needed or nor are not the same as those who actually carry out the assessment.
If they didn't think you met the criteria then they would passed your file back to the DWP as a fail, unlike ESA there is no requirement to hold an assessment for PIP before making no award.
