PIP Review Form 2025, new form layout was created by Seapearl
I'm starting to complete the new-layout PIP Review form AR1, for mental health. I'm planning to type my answers onto additional sheets and attach them, copying each activity on the form and each query within it, exactly as shown, followed by the answer.

Does anyone have any advice or experience in laying out the typed sheets in this new form layout? Or should I follow the previous PIP form style, essentially writing explanatory paragraphs for each activity?

Hi Seapearl

Do whatever feels right for you. There is no right or wrong way to do it. The only things that matter is that you put your name and national insurance number on each addition sheet - number all the sheets and make it clear which question you are answering (which is what you have already said). Also on the actual form on the part where it says have you anything else to tell us - write in there how many additional sheets you are attaching.
