Necessary to submit a diary for PIP review when same effect everyday?

I am going through the process of preparing my PIP review, and as it is for mostly mental health conditions, and as for my OCD I do not believe that this has been well documented since my last review (change of NHS area with worse services and thus no specialist mental health reports written on this condition), would I just be fine saying that some tasks are affected everyday, or that it takes me more than twice as long each time I attempt the task on no matter what day, or such things in a paragraph typed onto a word document that I can print as the extra pages to each section?

I ask this since one of my conditions, which is OCD, is very predictable generally day to day how it affects a given task (some other conditions may be the similar in this regard).

I am good at typing up descriptions, but I find charts difficult to complete, and I am wondering if I can communicate the same thing as a diary would by writing a paragraph that essentially gives the description of how much it affects me, how often (100% in the case of a structured repetitive condition such as OCD for example) and if I can do it reliably or if it takes longer than twice as long as the average person etc all in the description that I can write instead of using a diary?

The way to complete a PIP review form is to treat it as a new claim. You need to go to page 129 on the PIP guide to find details on the review.

A pip 1043 form is another name for an AR1 short review form, it should still say AR1 at the bottom left of each page. Its best to treat all reviews as you would a new claim.

If nothing has changed since your previous assessment, we would advise you not to just tick no change, its best to explain in detail what has changed and what has stayed the same and how you’re finding each task. If you kept a copy of your previous claim then use that as a template to complete your new review form. If there has been any changes in your condition(s)/medication then add the changes.

If you have an up to date review of your treatment, therapy and medication it could be helpful to include this also.

It is perfectly OK for you to type your answers and attach them as extra pages to the form, just make sure that each page has your full name and NINO at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

As to your question 'would I just be fine saying that some tasks are affected everyday, or that it takes me more than twice as long each time I attempt the task on no matter what day' yes it would fine to explain the difficulties you have carrying out some tasks.

I would not split your answers across the form and attached pages, do one or the other for each activity (not question), splitting it risks the second part of your answer not being read.

Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

If you feel you do not have enough time to complete this form you should be able to arrange an extension by calling the DWP and explain that you need a bit extra time.

Hi Gary, thanks for the detailed reply.

I never actually sent a completed PIP the past time I was awarded Higher Rate daily living and Lower Rate mobility (turned into higher rate mobility after I appealed showing a physio letter regarding my sciatica), because the mental health nurse spoke over the phone to the person from the DWP to make that review go from being originally a simple Lower Rate daily living only (from the assessment before that back in 2015, to suddenly a Higher Rate Daily Living and Lower Rate Mobility just after speaking to my Psychiatric Nurse on the phone for about 15mins.

So my point is that I didn't actually submit any form as my nurse just handled it on the phone back in 2018, and since then I have not had any physio since my pain was never getting better now for years, so no letter from any physio as they just stopped seeing me years ago, and I since I moved outside of London to another city in England, I have not been given any Secondary Mental Health here, so no paperwork on that other than when I had to be sectioned due to being seen as a threat to my own life, but likely due to cuts I never got a Mental Health specialist here like I had in London.

In other words, I am not sure what new evidence I can provide from doctors or nurses that are specific to the PIP descriptors.
Is my writing in detail, how my conditions and specific symptoms affects me in my daily life and mobility?
I mean, surely they would just view what I write in a form about my mental health as being as good as a doctor for things to do with how it affects me on a day to day basis? Since that is something the doctor or nurse would need to ask a patient anyway since they aren't with a patient 24/7 to know how they are affected. Or will I likely be seen as needing a medical assessment due to my lack of doctors or nurses notes relevant to my PIP descriptors?

As you didn't take part in filling out the form last time, I would do as Gary said and treat this as a new claim and tell them from your point of view how your conditions impact you. As it was the the psychiatric nurse who spoke on your behalf - you have no idea what has been recorded, and therefore, I would give them as much information as you can. Don't assume they will just go back to your last award and assume nothing has changed.

Hi, thanks for the replies.
I will go into detail on every activity then, likely as attached pages saying on the main form to see the attached page relevant section yeah?

Also, if I do that would I still need to fill in that diary template, or would describing it be adequate you think?

Yes, put on the main form where they can find your answers. eg "See answer on attached page 4"

You don't have to use the diary template if you don't want to.
