Requesting copies of completed pip form and advice was created by Andy
Hi all I Applied for pip on may the 1st 2015 due to my heart condition but on the same day i had a stroke I had made the call in the morning so telephone application was completed but due to my stroke I had to get age concern to fill in the rest of the form while asking me questions all went well and on the 20th of june had atos visit me at home for a face to face which went really well and had very nice man turn up .
now today the 2nd june ive been awarded lower rate care and lower rate mobility until 8th august 2015 after that date the rate goes up to higher rate care but still lower rate mobility the change is down to the stroke and having to wait the 3 month
well im not happy with it staying at the lower rate mobility as all my problems are mobility related and I can no longer leave the house unaided all this was put on the form and I also told atos at the face to face at my home yet they say I can stand unaided then walk 50m but no more than 200m
which may of been correct before my stroke but now I can't leave my home unaided so can anyone help me to find out how I get this changed or how to go about it ?
Also I have NO copies of any of the claim form as AGE CONCERN filled it in so how do I go about getting copies of the PIP completed form as I have no idea of what was actually put down so I concerned that they haven't put things down that should of been any help would be most appreciated
Thank You
Andy

Hi and Welcome to Benefits and Work
It appears that you have used your real name as your user name. If you wish to change this in order to maintain a degree of anonymity, you can do so easily by following the advice in this link
My full name is showing, how can I stop it?
If you are not happy with your award, your only course of action is to request a Mandatory Reconsideration. You can learn how to do this by reading our PIP guide. You should be aware that, although unusual, it is possible for your current award to reduce the award rather than increase it. I suggest you read our PIP guide and score yourself strictly against the criteria to see whether you fell your current award is secure. If you are not happy with the result of the MR you can then submit a formal appeal to the Tribunal Service.
You can obtain a copy of the report by phoning DWP and asking for it, although we suggest that you follow this up in writing so that there is evidence of your request. You can also submit a 'Subject Access Request' for a copy of all information that the DWP hold in relation to this claim. However, DWP have 40 calendar days to respond to this and you must be careful to submit your MR request within 1 month of the date on the decision letter. You can always submit a brief request and say that you will follow this up with more information when you have had access to the paperwork. However, we do have several members who have had the MR decision within days, long before the claimant has been able to submit further information.
