If a "medical" is deemed necessary, The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15 may phone you, but he should definitely receive a letter through the post confirming an appointment for a F2F.
If Maximus feel that they can make recommendations without the need for a F2F, they will return his file to the DWP with the recommendations.
The DWP ESA DM, "may" phone you regarding a decision, which would be more likely if they were considering a "fit for work" decision, but again you would definitely receive written notification of a decision.
Remember, he cannot be found fit for work unless he has first undergone a F2F "medical".

My last ESA support assessment was in December 2013 I never received an official letter (nothing new there) but did get notice via my MP that I would be reassessed in 2015, the time came and went without any letter or form so I decided to call the support centre for an update only to be informed my next assessment is now 2016 prob due to the backlog of new claimants
Its a good thing really as my dla is also to 2016 and both will be reviewed around the same time.

I am not aware of any significance.
All that I can say is there "should" most definitely be a recommended date for your reassessment on your file.
The ESA Call Responder should be able to access your file on-screen and inform you of your reassessment date.
Are you sure that The ESA Call Responder that you spoke to was not one of the many "numpty" Call Responders that we hear of from our members ?
I would be inclined to call again, and continue calling until I received the recommended date.

I have just called DWP again, the lady sounded very knowledgable and said that there is normally a review date on file, but after reading my file she said that the person I spoke to this morning was correct and there is no date on my file

Well, that is a first !!
You have "stumped the Mod" !!

I can only imagine that it is some form of admin error.
All ESA awards are theoretically "indefinite" but they are liable to regular periodic reassessments.
Under normal circumstances, (Without deferrals) the maximum recommended period (The Prognosis) is 3 years.
I cannot really offer any other advice other than to write to the address on your latest ESA letter, if you wish to push the issue :
Sending Documents to the DWP.
