hi
i thought you could request your previous award form but when i rang and asked he didnt have a clue what i was asking for and said that wasnt possible is this right ?

and the question on latest form about why and when it started i just dont understand what you are meant to write

Hi Dawn

I'm afraid you can't ask for the previous form, as they are no longer being issued.

Regarding the question "Please tell us why and when these needs began," - You can find an explanation on page 133 of the Guide to PIP Claims and Reviews with advice on how to answer this question.
