Do have a look at the guide. Although the review information is at the end of the guide, you can use the earlier information to answer the questions if you're struggling to do so There are sample answers for every question, and it tells you what areas you should include. You can use these examples as a basis for your answer.
Try not to be tempted just to say 'no change' - Even if there is no change, say why and repeat what your son still finds difficult or impossible.
Just do one question at a time, and you will get through it. If you're filling in the form for him, complete it in the third person eg "X is unable to cook for himself due to severe arthritis..."
