query on PIP review form

Hi I would appreciate some advice about how to takle the PIP review form ie AR1. In the Benefits and Advice guidance it states that it is important to address the following questions in each of the answers to the different activities in section 5: 1 What condition causes you problems? 2 What are the problems you have? 3 Do you use aids and appliances? 4 Should you have help from another person? 5 Is there a particular time of 6 Do your abilities vary? day when you have problems? Yet the questions in the PIP review form are variations on the following questions: • Can you manage this activity safely and without difficulty? • Do you need an aid? • Do you need prompting? • Do you need help from another person? • Please tell us why and when these needs began. Any advice on where to address your six questions with regard to the ones in the review form would greatly be appreciated. Many thanks
