I've helped my brother fill in his PIP form, and I'm confused about the declaration part. He won't touch the form as I've touched it (he has bad OCD). For the declaration signature part, do I write his name or my own?
He is supposed to sign it to say that it is accurate and there is a part for you to sign - if you filled it in. If he won't sign it -get him to write a letter saying why he can't touch the form with his signature on it. I would also get him to sign his signature on another bit of paper and then cut it out and glue it in the right spot and by the side of it - say see attached letter - to explain what you have done.
I would also include another letter from yourself explaining why you took this action.
Don't be tempted to sign the form for him - because it could be considered fraudulent.
