do I send in medical evidence with the UC form was created by Mojo7264

Hi there. Having to fill the form in online and there are no options to upload any evidence other than proof of ID and Address. Do I try to upload to my UC account?

Hi Dawn&herUke

When UC request information they normally put a To Do on your UC account with a link to enable you to upload docs. Or else you get a UC Journal message with instructions.
