As advised Ive taken a copy of my old pip review forms and I update supporting evidence and forms according to my issues. However this time the forms are substantially different to previous. For instance on eating and drinking they ask can you manage this activity safely and without difficulty, then do you need an aid (a box is then left for you to fill in) then Do you need prompting for example, reminding, encouraging or explaining by another person no or yes and then what do they do for you and how often? And then a box then Do you need help from another person? No or yes then what help do you need and how often do you need it? Then a box to fill in. Then Please tell us why and when these things began. It’s quite confusing as please tell us why and when (why???) and I have several conditions so this will take a lot of working out. What do Mods suggest for this? Ive got an extension to 31st Jan but this is making filling forms out so much more difficult. Unless I just try and add additional sheets answering the questions in one paragraph? Thanks in advance

Hi Chocolady
Don't try and restrict yourself to one paragraph per answer if it feels wrong. You should write as much as you need to. You can add as many additional pages as you wish. I would also add that many people won't be able to give exact dates - you just have to give a 'guestimate'
