I had my telephone assessment for PIP last week, but would like to submit a recent report on my condition as further evidence.

You might want to have a look at the following FAQ which explains where everything is; www.benefitsandwork.co.uk/guides-for-claimants/faq/forum.
The address to write to will be on the correspondence you have received from DWP, make sure you mark it for the attention of the decision maker and sent the extra information by recorded delivery.
