i received a letter in october 2020 extending the pip award to december 2021 this week i have been sent a review form to fill in, is this usual, and i cannot find a review form to download on your site, could you kindly give me a link thank you

And don't worry this is normal. You will find lots of information about reviews from page 102 in the PIP guide
