Does anyone have a link to the current PDF DLA application form that can be filled in via adobe ? I thought I had it, but its one you have print and complete by hand.

I am afraid it has been removed due to the introduction on PIP, only the child claim forms are still available online.
You can complete your answers in a suitable text document on your computer and write links on the form to the attached pages and vice versa. Make sure that any such pages are numbered, we recommend "page x of y" and that they all have your full name, date of birth and National Insurance Number.
