I have filled in this form and wrote 6 pages along with it.However,on page 6 number 1 question is 4 boxes this is about moving around and using steps
none of the boxes applies to me as I can only manage approx. 20metres-which boxes do I tick or can I leave it blank,I wrote words to explain in the box provide
the other one is going up or down two steps -I wrote I could but I could not do four steps as to repeat the task as I would be breathless etc-so which box should I tick
There are some very stupid questions on that form I really pity people,for it has taken me four days to get it done ,between my health problems. Thanks

Hi kv,
As with all the questions and their tick box options, if you feel that none of the tick box options are applicable to your own limitations, you don't have to tick any.
Instead you can simply write something along the lines of "None of the available tick box options are applicable, please see further information in the text box below." (Or, if on an added A4 : "on added A4 page number xx)
Make the ESA50 fit in with your own limitations rather than being restricted by the available options.
You might already be doing so, but if not, you might want to consider:
