Hello,

Its been a while since I have posted. My friend had received her Brown envelope a couple of weeks ago informing her that she would be going onto to ESA from IB, stupidly she did not tell me and only has till next Thursday to send it back. Luckily I asked her over the weekend if she had heard from DWP :ohmy:

Nevertheless, I have agreed to help her complete it to ensure that it goes off before the deadline of 26/9/13, but am in a bit of a panic myself, although I have not shown this to her.

I just wanted a bit of advise on where in the form should I put that she should be in the S/G under the Exceptional Circumstances, as she has a number of physical problems including chest pains which she has been referred to cardiology for and is on very strong medication which has very bad side effects.

Think about alluding to it in any of the ESA50 questions that touch upon these conditions, then include the full details and reasons in the Other Information section.

By the way if we are using the electronic version and I am typing the answers in her own words, does this count as form filled out by someone else????

If you are using there words throughout then use "I" and they can sign, if you at any time use your words then you need to use "he/she" and you need to sign as well.

In either case, explain how the form was completed.
