new esa50 and transfer from ib

On ib for more than12 years due to various health complications.
Last ib review 2008, without face to face medical, now recieved transfer new esa50 to complete
On page 1 it says SEND US ANY MEDICAL INFO YOU WANT US TO SEE.
1-- how many years (going back ) medical records should be sent, like hospital letters (consultants and physios and pain clinics etc.)
2-- some clinics use to attend discharged me about 2 years ago back to GP to continue medication prescribed by them due to hospitals cost cutting plans, should i send the previous consultantant letters of these clinics to give picture of diagnosis and treatment.

1. You could use pertinent historical letters going back some time to show that the medical conditions that you suffer, have in fact been diagnosed, and are chronic, long term and progressive, if that is the case. Try and sift out the important ones from general letters.
Always use copies, keep originals, you could highlight any important extracts and draw reference to them.
2. It is fairly common for Consultants, etc, to refer you back to the Primary Care of your G.P. once they feel that they can do no more, so again you could include relevant historical letters, as at 1.
You should be aware that even though historical evidence regarding your medical conditions should help, that you will be assessed on your capabilities at the time of this current assessment.
Moreover, it is the limitations as a result of your medical conditions that may or may not qualify you for an award of ESA with respect to the qualifying WRAG/SG Descriptors, that are more important than the medical conditions themselves.
If you could obtain more up to date letters, such as one from your G.P. explaining your current medical conditions and resulting limitations, that should help further.
