You need to speak to a qualified advisor about the options available to you as there are both short and long term implications that you need to be aware of before making any decisions.
Where to get advice?
In principle your husband can claim Income Support as a Carer, doing so should not affect your DLA, but you need to be aware the IS will be replaced by Universal Credit and the rules for Carers will change when this happens.
If you are found Fit for Work, it is possible that this will trigger a re-assessment of your DLA claim, it may not, but given that you are receiving the higher rate for both elements, I think it likely that it will happen. If you do not challenge the ESA Decision it may undermine your claim for DLA.

If you can clearly identify one medicine causing one side effect then you can go from there, but if it is a case that several medicines have the same side effects, or more likely, that you cannot specifically identify which is causing a problem, then just explain as best you can on the form.
Try and think of the problems in relation to the ESA Descriptors, so you mention problems with dizziness and balance, these would be applicable to the Mobilising and standing and Sitting Descriptors but probably not to the Communicating with Others one.

You could write you suffer from sleep deprivation due to your condition and/or medication which causes tiredness and fatigue and you would be a substantial risk at a work station
You would also be a substantial risk if you explained your balance problems , vertigo, dizzines etc and explain the restrictions and limitations
