I sent my form back on time but couldn't send GP's letter. I now have it but can't remember the
Address. It was a PO box in Chester. The internet gives different PO box numbers. Can I just send it directly to the Bootle address?
Any help would be appreciated.
Regards
Dave.
You should be able to get an address from the ATOS customer support.
Phone: 0800 2888 777 or 0113 230 9175
Email: customer-relations @ atoshealthcare.com (remove the space around the @)
You might also send a copy to the DWP, for the attention of the Decision Maker, at the DWP office that it is dealing with your claim.
Finally I would take a copy with you if you are required to have an assessment and give it to the HCP to read and include in your file.

Thanks for the information you gave me the other day, the extra material has now gone off to both ATOS and the Decision Maker.

I had a phone call from the DWP on Thursday, I wasn't too good. My wife answered the call and spoke to a nice lady who said "can you let your husband know everything will remain the same and a letter has been sent out" and then said goodbye and put the phone down. My wife rang back and asked them to explain a bit more, the answer was he has been placed in the "SUPPORT GROUP" for "3 YEARS". And without the need of a face to face.
Thanks to all here at B&W for your advice and guides, without them I definitely would have told them to stick it.

Just double checking to make sure I have the right info. It is the ESA85A I now have to request to find out the recomendations and how the DM came to his/her decision isn't it.
Thanks
Dave.
