on the last page of the questionnaire it states:
What to do next
1). You have answered all the questions on this questionnaire that apply to you
2). You have signed and dated the questionnaire
3). You return the questionnaire in the envelope provided with the original paper form we sent you or to the address on the letter that came with the paper form.
My question is regarding point 3).
Do you send the original form back along with the completed PDF Version?
It then states 'or to the address on the letter that came wth the paper form'
What does this mean?

If you have completed the "online" version of the ESA50 then you do not need to return the original (uncompleted) copy.
With regard to posting it, use the supplied envelope, see also
ESA50 too big for the envelope

In my previous post it clearly states the you have to send the original form back.
If you don't need to why would it be published on an official document?

It is instructing you to EITHER return the questionnaire in the pre-paid envelope that arrived with your paper version ESA50, OR in another suitable envelope/package to the address that appeared on the accompanying letter that came with the paper ESA50 through the post.
You are reading, : "You return the questionnaire in the envelope provided with the original paper form we sent you"
as, You return the questionnaire in the envelope provided, AS WELL AS (with) the original paper form, when it actually means "the envelope provided THAT ACCOMPANIED (with) the original paper form.
The benefit of using the pre-paid envelope provided, or sticking it on the front of another larger enevelope is regarding postage costs, as explained in :
ESA50 too big for the envelope
It is not telling you that you must return the pink paper ESA50 that came through the post, even if it has not been used.
Please be assured that you do not have to return the unused pink ESA50.

I think that you are misinterpreting the instructions at 3).

It is instructing you to EITHER return the questionnaire in the pre-paid envelope that arrived with your paper version ESA50, OR in another suitable envelope/package to the address that appeared on the accompanying letter that came with the paper ESA50 through the post.

Another question I have:
I have a Medical Evidence letter from my GP, it is Dated 27TH June 2012.
My question is would this be accepted along with my ESA50?
The reason for asking is the date of the letter which is 10-12 weeks old.
