You can refuse any physical test that would cause you severe discomfort.im having a steroid injection into my thumb arthritis tom morning so also worried the medical might make me do movements which would aggravate my hand as i seem to have a strong reaction to steroid jabsamd the pain is intense for a week after the jabs. i suppose what im saying is can i refuse tests if i know they will aggravate my symptoms or do you hvr to do them even if it makes me worse?
Most people guage their capabilities by their pain, whilst there is no specific definition of severe discomfort, it is definitely less than pain, so you should adjust what you think you can do accordingly.
If you need to refuse an activity, explain why you are doing so, be as detailed as you feel you need to be.

Firstly I wish you good luck and hope you find some inspiration today / tomorrow to help you.
The Dr is not supposed to ask you to do anything that will cause you pain or distress but if it was me I would make the minimum amount of movement that I could even if very insignificant before explaining what pain and stree any more would cause you. I understand you can refuse to anything that would cause you pain and grief but I would not want any "refused to do" on my medical despite a detailed expanation as it may not be written down.
I understand there are just some things that people cannot do but if you can move your finger, hand, arm an inch then that is what I would do and then explain the pain going any further. To me you have attempted exactly what is asked of you before the onset of severe pain / discomfort.
If you cannot move a muscle then I feel for you and as others have said you need to explain and maybe ask to see the paper to see they have written it down for you.
Good luck tomorrow, I hope it ocmes and goes without incident and worry for you.

Every time I have steroid jabs in joints the hospital give me an advice leaflet and one of the important points is not to put the joint under any stress for the first 48 hours. Ask for printed advice leaflet at hospital and produce it at medical if necessary, to show you are not just being awkward about not doing a certain movement. It's important, as I messed my first thumb joint jab up by putting the joint under pressure the same day, this was the time they forgot to give me advice sheet!
I hope your medical goes OK for you but don't be afraid to groan or voice your feelings about any pain during it. People who try to put a brave face on often get denied their rightful benefits!
