If been in Income based ESA Support Group but not had a reassessment for a number of years, will I have to be reassessed as part of the migration to Universal Credit

No, you won't have to be reassessed as part of the migration to Universal Credit - they are just trying to get the admit sorted as they move people across.

Ref: The Migration form -
Nationality, it asks if I have been out of the country in the last two years. Even though I have answered that I am British, do I have to include holidays taken abroad in the last two years as being "out of the country"?

I would include holiday(s) if you have been abroad, they will now all this information from your passport.
