Do you have a Social Landlord? If so they usually have a Financial Inclusion officer who could help you with tenancy related issues and maybe in getting some personal support.

Hi David,
I believe so. But I try to avoid them at all costs as they are a absolute nightmare to deal with. They would be the last people in the world I would goto for any kind of support.
Am more concerned about the other questions I don't know what to do about.
2) On the migration letter it states that my HB will continue to be paid directly to my local council because I am in supported or temporary accommodation. This is not the case though but I do rent from a housing association. It seems the DWP have been given the wrong information though obviously presently my HB is paid directly to the local council as am claiming ESA.
Should I call my local council and get them to change this?
Or should I mention it in when making the UC application? Or both?
Perhaps I need to call the DWP on the UC & ESA number to let them know?
5) More importantly, my next question is regarding proof of my conditions.
Due to my conditions depression, anxiety, agoraphobia etc. not seen my GP since Feb 2020 (also because of covid / agoraphobia), not seen the mental health team since 2017 & I’ve not been in touch with my social worker since last August again due to severe depression.
I have no idea where any of my documents are due to the above.
Other than trying again to contact my GP for a telephone appointment and/or home visit and getting in touch with my social worker what else can I do to obtain proof and evidence of my conditions?
Can I get access to all my mental health records and NHS health records, and records from the social welfare team?

I know that you have difficulties with your Social Landlord but unfortunately as a tenant you are going to have to talk to them to sort this issue out. The Social Landlord has a portal with Universal Credit for sharing tenancy information. Obviously they have to contact UC with the correct tenancy details.
You have a right to a copy of your medical records under what is known as a SAR Subject Access Request. You need to contact the health provider directly to engage in that process.
