In the Name of condition box (one per box)it allows you to put in another condition but could anyone who has answered this question advise me if the system would allow twelve conditions to be entered.
I could do this with PIP paper form but i am unclear with this online UC Application Form.
I would prefer full disclosure as I do with PIP or any medical insurance.
If it is restricted should I prioritise what affects me most and leave out the rest of the conditions. Perhaps mentioning this when I have a journal set up.
