  			 				Hi, I wonder if anyone has come across this before.  I had my Pip telephone assessment 3 weeks ago. Within an hour I had a text to say the report had been submitted to DWP. I waited a week or so as advised by your good selves  before requesting the assessors notes. As seems to be the usual case they are full of inaccuracies as well as very important information being completely left out which I plan to inform DWP about.
What I also noticed was the assessor refers to two activities Preparing Food and Washing and Bathing as being awarded 1B and 4B respectively in my previous assessment when in fact I was awarded 1E and 4E after MR.
Should this inaccurate information make a difference to the possible outcome given by the decision maker? My condition is considerably worse having severe pain and lack of mobility from complex spinal conditions, chronic arthritis which affects most of my joints. I’ve had a total right shoulder replacement which helps with the pain but will never give me normal/ full mobility, I will also need to have the left shoulder replaced. I’m waiting for a hip replacement and also have osteoporosis. I’m trying to stay positive but my anxiety about it all is through the roof. Any advice is greatly appreciated.

I am reading so many of these types of posts this morning about inaccurate information sadly. Best to obtain a copy of the PA4, and list the inaccuracies, and put in an MR, listing how your different conditions affect you on a daily basis.
