Figures for the number/% of 'outcomes' DWP get WRONG - i.e. appeal is UPHELD?

Hello,
I am a disabled CLP Disabilities Officer and I am trying to put together a response to the Govt/DWP propoganda about 'FRAUD' in the Benefits Process.
The Bill they have recently submitted includes the wording 'FRAUD and ERROR', but it is nigh impossible to find anything about DWP Error !
As we will all know, the DWP 'assessors' seem to BY DEFAULT find us 'fit for work' and leave it up to us - with the incredible help of this amazing website - to lodge an appeal to get the 'outcome; corrected.
Surely successful appeals should be considered 'DWP Errors' ?!?
I would be extremely grateful if the kind people of B&W could provide me with such figures (and include links to sources, if possible).
