Sorry to intrude on this thread. When filling out a review form and using separate sheets. Should you attach the extra additional info at the back of the form itself or after each question.

I moved your post so we don't create a separate conversation on someone else's post.
There are no rules about where you attach the addition sheets. Personally, I would put them at the end, but it is up to the individual. The important thing is for them to be marked clearly.

Hi AA,
Whatever works best for you. I filled mine in myself and it was rejected both times. I eventually got someone to help, then won my appeal. It's all in the wording, rather than who writes what.
