Well done Steffi. Having written this on your form, just draw the DM's attention to it in your MR request. Also tie this in with any supporting documentation you have sent.

For instance - I stated x,y,z on page a of my PIP form and it is supported by a letter from b on page c of my supplimentary sheets.
