ESA3 form

I am told I need to complete an esa3 form. - has any one filled in this form - could you please give me an idea of what the questions are ie are they all financial or medical and will I have to attend another assessment or will it trigger an earlier reassessment than the proposed.

the idea of these forms makes my stomach churn
