Hi Steve
Normally refering to the tribunal means the upper tier tribunal after going through MR and first level appeal.
Tou can get the form from the appeals office that sent out the decision of the appeal panel. Upper tier tribunals are generally on points of law and you are also entitled to legal aid for this level of appeal.
