Requesting A Copy Of the HPs report (PIP)

Following my assessment of PIP yesterday I wanted to request a copy the report the health practitioner wrote about me. Does anyone know how I go about this?
I have been looking through the booklets on the website and in my paperwork but I cant seem to find any instructions on how to do this.
Sorry if this has already been covered. I read things then either cant understand / take them in or forget them!

Just ring the DWP office dealing with your claim and ask for a copy, I would follow up with a written request.

She then went on to say that I would have to have an assesment done by dwp to see if I actually needed an appointee and that they could send someone round to my house. My carer said there was no need as I was standing next to her and she could put the call on speaker phone so that I could confirm it was ok to have someone speak on my behalf (the reason I dont speak on the phone is because I cant remember conversations and often cant process what they are saying and I've already written them a letter saying this). DWP woman didn't even respond to this.
The conversation ended with her saying I should have a decision about PIP by the end of next week. That's a surprise as they told me 8 weeks I"d have to wait. But there you go.
I am now going to write a letter asking for the copy of the consultation.

The DWP are releasing copies of the assessment report before a Decision on the claim is made, it is possible that they have now changed this policy, it didn't make much sense in the first place, so you may want try again.
Someone can speak on your behalf on the phone, without being an appointee, but they need to explain this at the start f the call and confirm that you, the claimant, is available to answer the security questions and to authorise the call.

I thought this was the case when other people phone on your behalf. I had a social worker a few years ago and he never had any problem phoning the dwp for me. I was always there and like you say they can ask you security questions. Perhaps on this occasion we weren't clear enough.
