If you are going into hospital then you might want to ask for a two week extension to the form deadline, give the DWP office dealing with your claim a call.
PIP will be awarded on how you have been effected for the last three months and how you reasonably will be for the next nine months, so your surgery will certainly complicate matters and may delay a Decision on your claim.
As you have not had the surgery and are unlikely to have any realistic view of whether it has been successful or not before your form needs to be returned I would say that you can only discuss how you are limited now, but you do need make it clear that you are having this surgery.
Be careful in regard to the non-mobility activities as your ability to walk, stand and raise yourself may well be considered in some of these; for example; preparing and cooking a meal, washing and dressing. If your problems are not related to your legs then you need to make it clear that this is the case to prevent the assessor and DM assuming that it is and downgrading you because of this.

Why do they ask you if you will be filling out the form yourself ????? or will you be receiving help ,

Your DLA will remain in payment until four weeks after a Decision is made on your PIP claim, if you have a fixed term DLA award then it will be extended to accommodate this.
Some claimants are unable to complete the form for themselves, there can be a variety of reasons for this, a physical problem which prevents them writing or typing or perhaps mental health issues where they are not fully aware of their limitations or they may have an appointee who deals with these matters for them.
