what evidence to send with pip application?

hi,
I was successfully transferred from incapacity to esa support 2 years ago without a medical. we asked for copy of the report and was sent not only their report but copy of my form and all the evidence I sent.
I am just thinking about the ending of dla, which has started in my area and having to apply for pip. I was wondering has anyone has sent in with there pip application a copy of the form they completed for esa and all evidence supplied with it, as such a lot overlaps??

my esa report letter states........ med 3, esa50 , and medical evidence suggest client cannot mobilise 50 mts repeatedly within a reasonable time scale and unaded by another person because of significant discomfort or exhaustion due to multiple co morbidities. has limited capability for work and work related activity.
experiences severe overall body pain, under care of multiple specialists exacerbated by other chronic health conditions.
would be unable to self propel wheelchair due to chronic pain issues.
the available evidence suggests improvement is unlikely in the longer term.

my daughter completed the esa form and typed in my answers found the relivant consultant reports from over many years ( paid to get my medical records from my gp practice) and covering all the questions asked on the esa form. in all there were about 45 extra pages of reports from over 20 specialists. she is now abroad and not here to help me and I am completely unable unable to cope with completing whatever form im sent. I panic just thinking about it. any advice, or other peoples experiences

Yes you can send in the ESA report, however if it is now two years old then it will not carry much weight even though it specifies improvement is unlikely in the longer term, due to it's age, assessors and Decision Makers are always optimistic as to the prognosis that a claimant might have.

So, you should give some thought as to what other evidence you might source to support a claim. A good place to start would be to have a conversation with your GP about the situation to see what assistance they might provide and then go from there.

You should also have a good read of the PIP Claim guide to make sure that you fully understand the criteria that your would be assessed against.

our gp practice opted out of providing ANY info , advice, or letters stating health conditions etc for either esa , dla, or pip, so no help asking my gp. would it be helpful as well as sending in my medical report for esa, if I sent all the consultant letters I included with that claim, were they cover questions on the pip form, eg mobility?
and just add anything new I have, which isn't a great deal, the consultants reports cover 1985 to 2013, thanks
