Hi guys, some help please. I will try to keep it short but it is a bit long winded.
I have had 2 periods of DLA, this current one expires end of Jan 2016. Last Friday I had a letter from DLA saying I need to claim PIP, within 4 weeks of the date of letter. As I did not have a PIP 2 in the original letter I phoned and ended up having my telephone "interview". I was asked if they could transfer my paperwork from DLA, I agreed. I was also told they would post PIP2 form to me. Today I received a letter saying they have got my paper work from DLA but the is no supporting medical evidence. It was all there as I have been awarded DLA on 2 previous occasions, both at first attempt, so it is conveniently lost. Still no PIP2 form. Since my lattest DLA I have been homeless for over 2 years and have had all my possessions stolen, so I do not have copies of the evidence. I am now with a different doctor, only 3 months, so I don't really have much history with them . I spoke with the receptionist and asked if I could get a summary of my problems but she wasn't that helpful. Any idea's what I can do ? and how long should I wait for the PIP2 form.

Welcome to the forum.

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

There are a couple of things that you can do.

First I would raise a Subject Access Request (SAR) with the DWP for the documentation they are holding on from your DLA claims, if they have indeed misplaced it, then your request may come back empty, but you may still find some information that would be helpful to your PIP claim.

Secondly you can request copies of your medical records.

The PIP2 form should take no more than two weeks from the date of the call, so if it has been longer then you should contact the DWP about this.

Even though you are new to this GP, they should have received the envelope containing your old paper records. This should include letters from consultants etc.

If I make the request will they send me a copy of the DLA form I completed last time, along with any notes they may have ?. despite sending me a letter stating they have no medical evidence. I spoke to the PIP office this afternoon but even the supervisor could not answer my questions, so she is getting someone to call me, within 5 days........................ I am not going to hold my breath. A question I asked was............ if you make a decision on my PIP claim before my DLA award finishes and that decision goes against me will DLA continue to the original claim end date ? She only THINKS not and it would end with the decision but when I asked if the decision came after the claim end date, would they continue to pay me until a decision was made, she din not know the answer. I think I can make an educated guess.

You can specify your previous DLA forms on the SAR and if they have them then it should be included in their response.

Whatever happens with your PIP claim your current DLA award will stop four weeks after the Decision on your PIP claim is made, if necessary, your DLA claim will be extended if the PIP one takes longer than expected.

I had my "telephone" interview 11th Sept, spoke to PIP on 18th, re my medical records, they couldn't tell me where my records would be. I spoke with a team leader, who still didn't know what would/should happen to medical records. Team leader said she would get someone from benefits to call me. She told me that my PIP2 form had not yet been sent.
I had a call from benefits office on Monday 21st, I was told that after one year all medical records are destroyed, shredded as confidential waste as after 12 months they no longer are current. I told the caller I still had not received my PIP2 form, was told it was posted on 10th Sept and they would send me another. It seems that nobody there has any idea of what is and what should be happening. I am starting to worry as the PIP2 form is still not here.
