Care needed for mental health condition

I know where to put the care needed when out for safety reasons etc... but where do you write about the care indoors and throughout the day that is needed to keep you safe? The monitoring a health condition question just seems to be about medications.
What if you need help to stop self harming or being a suicide risk indoors and throughout the night, where does it ask about that?

Unlike DLA, PIP does not directly consider a claimants care needs, you will need to explain the assistance and support you need in regard to each of the PIP Descriptors, as relevant, think about the triggers that might cause you to self harm, as an example; if one of the reasons you might hurt yourself were due to your stressing about meeting other people then you could explain this in question 11 Mixing With Other People.
