Hi,
I was diagnosed with Crohns Colitis around 1998, I have always worked & never claimed any benefits since I left school at the age of sixteen.
Last year having gone through a dispute with work regarding various issues relating to redeployment & discrimination, the outcome was they paid me £9,000.00 and termination of my post with them.
Initially I was placed on Job Seekers Allowance then I explained that the main reason I was paid off from my former employees was due to them not supporting my health issues by taking my right to work flexible hours away from me.
I explained that it had gone on for 2 years & the dreadful impact it had caused me & still is.
I was then put on ESA & it's always a problem getting the sick note from my GP.
So at times if the sick note was late I was left unpaid but as I am married they were not concerned even though I was on the contribution ESA.
That has now finished & I have been asked to fill in a re-assesment form, I am concerned as I do want to go back to work but very anxious that due to my Crohns which is a disability I believe, I am going to have history repeat itself .
Could someone guide me on how I should fill the form out so that hopefully I can also be seen as disabled . So upon my return to work this will be supported.
I look forward to the advice.

From what you have said, your ESA payment has ceased due to the time limiting of the Contributory benefit?
The ESA3 form is purely looking at your financial situation to see if payment can continue on an Income-Related basis. This from does not ask for details of your health or assess your medical entitlement to ESA. This is done via an ESA50 form, have you had one of these yet? It sounds as if you might still be in the assessment phase if you are still required to send in fitnotes?
