DLA claim form 2009

Can you signpost me to a copy of the official DLA claim form from 2009. I've searched the net but my concentration is not so good, and around in circles I go. Thanks.

The DLA Adult Claim forms are no longer available, however, I'm not sure why you would want one, from your previous posts you are in Lincolnshire which is an active postcode and any renewal of your DLA will result in a invitation to claim PIP.

Yes, i'm in an active PIP (Capita) area. I don't want to make a claim or renewal. When I last completed a DLA claim form it was 2009 (indefinite low care & mobility since), but I didn't make a copy so don't remember all the details that were put on the form, and a professional wrote the answers in for me.
I just wanted to read the questions on the form to see if any memories of the answers that were given are sparked. I have a very damaged memory (TBI), so it was a bit of a long shot anyway, and as you say, "The DLA Adult Claim forms are no longer available". Was hoping someone had an old one lying around.

The PIP 2 "How your Disability affects You" Form is totally different anyway.
