I am not aware of any recent statistics for this, my best guess is that the revision rate is at best around 10%. There is clearly some variation between benefits, PIP appears to have a higher rate than ESA.

As to your second question, DMs are required to justify a Decision not to revise, so one has to assume that they look at the MR properly.
