Advice on PIP please and Special Rules

I have previously been on DLA. And then Special Rules since October 2012, received letter saying have to transfer to PIP. When I rang I said I was already on SP so what to do now, DWP says claim again on SP BUT had moved home 2months earlier and changed GP practice, none of them knowing me they refused to sign another DS1500, also they were concerned that GP's only get paid once for signing DS1500????
So I claimed PIP using you excellent guide sent form in and ATOS are doing a home visit on 02/06/2015.
Yesterday after speaking to the pallative care team my GP, who I now know a little better, says she has signed the DS1500 and has sent me the Patients Copy!!
What do I do now ? Do I have to send this copy to DWP or ATOS ?
I am a loss as to where I am with this claim, does the GP automatically send her form to DWP?
I think half the problem is that I have lived longer than any of the professionals thought I would and this has confused them all.
Can anyone advise me of what will happen now or what I should do???

You need to send the form to the DWP, I believe that the form needs to go to the following address but it would be worth your contacting them to confirm it as they regularly move things around

There should be a contact number on the last letter that you received from them.

It would also be worth your while to contact ATOS and inform them that a DS1500 has been issued as this may mean that they will cancel the requirement for an assessment.

The claim form has already been sent, from this I got a letter saying it had been sent to ATOS, from ATOS I have the appointment on the 16/06/2015, so should I telephone ATOS or DWP?

Both! The DWP in regard to where to send the DS1500 and ATOS in regard to the assessment.
