ESA50 form filling

Hi,
wondered if you could give me some advice on filling in the dreaded ESA50 on behalf on someone else? (they have major dexterity issues). Would you complete the form in terms of "my wife/son/daughter etc " "he/she is unable" "they suffer from", or is it best to fill in the form as they had wrote it themselves but pointing out the need for someone else to fill the form in?

There is no power of attorney, or anything of that kind, they just have issues with both hands making it very difficult & sore to write. Signing the form should not be an issue.

Any advice would be appreciated

If you are completing the form using their words, as seems to be the case, then you should "I" as if they have written it, however, you should include a note saying what you have done in the "further information" section and you should explain in detail in the Manual Dexterity section why the claimant was unable to complete the form themselves.

Finally, the claimant should sign the form if they are able to, if not then you should sign it "on behalf of".
