In my MR I included reports from surgeon and GP confirming in unable to push myself in a wheelchair and cannot walk 25 meters, im suffering with chronic pain have limitations because of my condition, my GP has even confirmed this to them in a telephone conversation. Have you any idea why I have had this form?

Filling in and returning The ESA 2 Form may be beneficial.
As with the very similar ESA 3 Form it is used to financially assess you for entitlement to payment of Income Related (IR) ESA which can be paid alone or on top of Contribution Based (CB) ESA.
The ESA 2/ESA 3 are not used to reassess which "group" you are in. (WRAG or SG) the ESA50 is used fro that.

I see that you have been in The SG, if you have not been receiving The Enhanced Disability Premium, (EDP) (currently £15.75/Week) you may be entitled to it from the date of your SG award if you are also entitled to IR ESA Premiums
Being paid IR ESA, even a penny can passport you to other "benefits" !!

My best advice would be to fill it in and return it to the address provided.
