DLA to PIP was created by Katykat
I care low care DLA indefinate but my condition has deteriorated and I am considering informing DWP. I didn't have a medical when awarded DLA but I am aware that I will have one if I change to PIP. How do I go about it? Is there a particular form I ask for?

The first thing you need to check is whether your postcode is active in regard to PIP, if it is not then any reported change of circumstances will result in a DLA re-assessment, If you are then it will be an invitation to claim PIP. See

Next you need to have a look at our PIP Claim guides to see whether your worsened conditions will result in a PIP award, you need to mark yourself harshly. The criteria for PIP are very different from those for DLA, whilst they favour some claimants, the majority will find it much more difficult to qualify for an award.

If you still think that you would receive an award then you just need to notify the DWP office that deals with your DLA claim that you are reporting a Change of Circumstances due to a deterioration in your health.
