Am I correct in believing that if you think that you may be entitled to a higher rate of mobility and or care due to a deterioration in your health since the initial award of DLA (which may have been some time ago), that the only way to do this is by informing DLA of these changes in your health condition ?

Yes, that is correct !
However, you should be aware that The DWP would most probably look at the whole of your existing DLA awards afresh.
Another important issue to take into account before reporting a Change of Circumstances (C of C) to DLA is that if you are in an Active PIP Postcode Area, the review carried out as a result of reporting The C of C would be under PIP qualifying criteria, rather than DLA.
