ESA reassessment after a computer glitch at DWP

Just to let you know that I phoned both offices this morning with this result.
CHDA will post me a letter confirming that my assessment is going to be recorded!
The address for CHDA to where the documents should be sent is:
Centre for Health and Disability Assessments
Wembley ASC
Mail Handling Site A
Wolverhampton
WV98 1SJ
They said as long as I put my name, NI (and guess DOB) on the documents I'm sending, it will get to the right person, as they all deal with the assessments.
The address for The ESA Decision Maker is in Belfast, apparently, because of where I live. They didn't ask me any questions apart from my postcode in order to give me the right address. I questioned them about the address on my recent benefits letter that is different, but they said it didn't matter, as all mail (documents/forms) go to a specific address depending on the area where we live.
I've forgotten to ask you, if you think I should (apart from the covering letters) send my hospital letters/notes with the forms to both sites or just take it with me to the assessment? The more the better?
It was good to read the additional posts about deferrals, as it seems that I may've been in that category...
BTW, further to lost documents and ESA50 forms 'not' needed - I looked at my ESA85 form again and it says there that ESA50 for my last assessment was not provided!! It was, of course.

Yes it would be fine to send copies of any extra supportive evidence along with The ESA50.
