It does happen but only very rarely, usually where the claimant is known to mental health issues.

There are two way for you to approach the situation, depending on your circumstances and the time available.

First you can complete an ESA50, there is a version that can be downloaded, in the hope of providing a complete picture of your limitations and getting the right Decision from the outset.

If there is not enough time or you do not feel that you can provide an adequate description of your problems within the available time, a badly completed ESA50 can do more harm than not having one, then you can go to the assessment without one. The risk is that the assessor understates your problems on their report, but not having completed an ESA50 could be an advantage if you need to challenge the Decision.

Of course, the ideal situation is that the right Decision is made first time.
