Its here the big brown envelope landed on the mat today got the renewal forms i been waiting for from DWP only its not its a apply again from scratch??
I slightly worried now as to they don't have the old paper work says about all my husbands illnesses the problem is we have got a new doctor not where i can go to get help with doing the paper work i not sure if i can get everything over to them as it really is. he has ME, PARKINSON, and loads of other stuff
i understand that there is loads of notes on here to help but because of my arthritis i have trouble writing i i could do it on a PC it would be better,But i cant so i would really like to find out if there is anyone like DIAL as ours has now closed down.

From your previous posts I believe this is for a DLA renewal.
I am afraid that there are no DLA for adults forms online due to the introduction of PIP, you can however, prepare the bulk of your content on you computer and attach it to the form, ensuring that you include references to and from each question on the form, each additional page needs to have your husbands name and National Insurance Number.
This will be treated as in effect a new claim for DLA so you need to fully explain his care and mobility needs and provide whatever evidence you have available. Have a look at our current DLA guides for more information.

If you are applying for a renewal of your ESA, then you can download a copy of the form and fill it in on your computer. You can even fill in a bit, save it and then come back the next day and do a bit more. Then, when you've finished you print off the final version, keep you own copy for reference next time, sign the one for the DWP and send it off in the envelope provided. All you need to do is make photocopies of any letters from your doctors, keeping the originals for yourself next time.
And of course you can read all B&W's fabulous guides on the computer - including all the stuff about which form to download and how to deal with filling it in on your screen.

Thank you Gordon i am just a little worried about the new doctor whom we have seen only twice,
In fact the last good evidence we got from anybody was in deed from DWP its self as my husband had to go to a DWP medical and the nurse told us that should could not carry on because o my husbands Parkinson's disease and this we didn't even know about ( we then got hold of our doctor) so i really not sure what do i need help
