which is the best Benefits and Work guide to use please?

As far as I know, no! They were all removed when PIP was introduced.

You can attached additional pages that you have created on your computer, just make sure that they are clearly labelled with your full name and National Insurance Number and that there is some form of page reference, we recommend "page X of Y" or something similar. Also you need to make sure that there are clear references from each question on the form to your pages and on your pages from the form (i.e. "continued on page X" and "continued from Question Y").
