hi,this may sound silly,but i have so little confidence when having to speak to someone,i would like to veiw my medical records & i would like to explain that i need them for my new pip claim,i know i dont have to give a reason, but i would like to,only how do i phrase it?? "DWP have asked me to provide dates began for each health problem that i have ,Got it wrong somewhere? feel so dumb on times,Sorry..

As you say, you do not have to give a reason, as it is your legal right to make a Subject Access Request (SAR) for your G.P. records under The Data Protection Act 1998. (DPA 1998)

You simply make the request at your G.P. Practice reception, there will be a form that you need to sign, and there will probably be a charge.

If you wish, you can simply state that you need the information to support a claim for benefits.

G.P. practices should be used to such requests, as many claimants are having to request this information in support of a benefit claim.

All G.P. Practices have people that come in to deal with such requests. Depending on how they deal with it, they may well provide you with a password protected CD which will have all your records on.

You can then view the CD, and print off any relevant extracts.

If you have any problems, ask to speak to the Practice Manager.

If you wish to obtain any medical information held by a Hospital where you have received treatment, you should approach The Patient Advice and Liaison Service, (PALS) they should be able to point you in the right direction.

Any letters written by a Consultant to your G.P. after an Outpatient Clinic appointment, should be in your G.P. records.

However, for future reference, if you are attending any Consultant's Clinics, you can make a verbal request that any post clinic letters are also copied in to you, as well as your G.P.

There are no forms to fill in for this, and no charge.
