End of year 1 in PIP

I have now received a 'How your disability affects you' form through the post. It's as if I was making a fresh claim for PIP.
I do have a copy of my original form from last year, do I simply complete this new form repeating word for word? I would appreciate any advice from anyone who has been through this process, or any advice from anyone.

I believe that we are pursuing this with a Freedom Of Information request but as far I am aware we have not yet received a response.
Yours must have been one of the very first PIP awards as we saw no more than a handful in 2013, your certainly the first who has reported receiving this letter!
In the absence of further information I can only recommend that you complete this form in as much detail as you did for your original PIP claim, the DWP may well be within there rights to change your award if they believe your circumstances have changed.
If you are able to, we would appreciate a copy of this form, if you could email or post a copy to us it would be much appreciated. Is there a form number, this may allow us to find it ourselves?

There is a whole string of numbers at the bottom of the first page, they are:
0000000075771400001160
00043/000007/000011
The bottom number only, continues throughout the rest of the form.
The format is the same as the form I completed a year ago where you tear of the front page as it doesn't need to be returned. The only difference is the beginning of the letter where it says "We're looking again at your Personal Independence Payment"
How can I email a copy to you?

If the format is similar to the PIP2 you originally completed then you should be OK to use the PIP Claim guide to help you fill it in
